A symptom tracker is a tool designed to help you manage and monitor your symptoms as they change over time. Keeping track of your symptoms can help you understand what is triggering changes, or how your environment or lifestyle might impact your symptoms.
Symptom tracking can be especially helpful for you if you are managing chronic health conditions, especially those with a wide variety of symptoms associated, like mental health conditions. Symptom tracking can also be helpful for you, even if you don't have a diagnosis of a chronic condition so that you can share better health information with your healthcare providers to get better treatment or receive a diagnosis.

“I have never taken a questionnaire like the symptoms survey in Human Health, and i wish it would be available in more clinics and counselling centers. It truly made me feel seen and heard, and that i am not alone.”

The Symptom Tracker lets you record the symptoms that affect your life, whether they are physical, mental, or emotional. Each time you log, you can rate severity on a simple scale and add notes about what was happening around the time.
Over weeks and months, this builds a timeline of your health that helps you see what is changing, instead of trying to recall it all from memory. Many people use it as a daily health diary that becomes especially useful during medical appointments.
The Human Health app covers over 1,500 symptoms across more than 1,000 conditions. You can track everything from fatigue and joint pain to brain fog, anxiety, or nausea. If something you want to log is not already listed, you can also create a custom entry, so the app adapts to your needs.
When you log consistently, the app organizes your symptoms into charts and summaries. You might notice that flares happen after stressful weeks, during poor sleep, or when treatments are inconsistent. Even if the app does not prove a cause, these records can reveal patterns you may not have spotted otherwise, giving you more insight to discuss with your doctor.
Paper logs and spreadsheets can be hard to keep consistent, and they do not automatically organize data over time. With Human Health, your logs are securely stored, time-stamped, and displayed in charts that make trends easier to see. You can also export a PDF summary to bring to appointments, which is much simpler than sharing handwritten notes.
Yes. The Symptom Tracker is included in the free Human Health app. You can log symptoms, add notes, set reminders, and generate summaries without paying. Optional premium features are available, but the core tools for tracking symptoms are free to everyone.
